Correct Fan for 68 Cougar 390 Non AC 4 speed?

What is the correct Fan Ford part number for 68 Cougar 390 Non AC 4 speed? Would it be 5 or 7 blade? I also need the spacer part number?
I don’t think it came with a fan clutch. Is that correct?
